Participants & Residents (Currently receiving Section 8 or Public Housing Assistance)
Title	Purpose
Authorization For Release of Information	To allow UHA to speak with other agencies regarding your housing assistance.
Change of Income	Your income has changed (increased OR decreased).
Request For Appointment	You need an appointment with a UHA staff member.
Supplement to Application for Federally Assisted Housing (Form HUD-92066)	To designate an individual or agency who can assist you with your Public Housing or Housing Choice Voucher (Section 8) assistance.
Applicants (Persons on the Waiting List)
Title	Purpose
Authorization For Release of Information	To allow UHA to speak with other agencies regarding your housing assistance application.
Notice of Change to Contact Information	Complete this form if:  - You have moved  - Your telephone number has changed
Request For Waiting List Status	To find out if you are active on UHA's Public Housing or Section 8 waiting list.

Property Owners & Managers
Title	Purpose
Statement of Property Ownership	For owners & managers to prove ownership of the property they wish to rent to a Section 8 participant.
W-9 IRS Form	For owners & managers to report their tax ID number to UHA.
Rent Comparibility Survey	For owners & managers to report unit pricing & amenities for the purpose of rent comparison.
Notice of Landlords' Rights and Responsibilities Under the Violence Against Women Act (VAWA)	A summary of landlords' rights and responsibilities under the Violence Against Women Act (VAWA).
Tenancy Addendum Form HUD-91067	Required to be attached to all leases in accordance with the Violence Against Women Act (VAWA).
Certification of Domestic Violence, Dating Violence, or Stalking (Form HUD-91066)	Landlords may provide this form to a tenant who is a victim of domestic violence protected under the Violence Against Women Act to document the circumstances of the situation.

Notice to Public Housing Residents	A summary of Public Housing tenants' rights and responsibilities under the Violence Against Women Act (VAWA).
Notice to Section 8 Participants	A summary of Section 8 participants' rights and responsibilities under the Violence Against Women Act (VAWA).
Certification of Domestic Violence, Dating Violence, or Stalking (Form HUD-50066)  - English  - Spanish  - Vietnamese	For victims of domestic violence to document the circumstances of their situation when requested by the Housing Authority.
